Wegovy®(Semaglutide) or Zepbound® (Tirzepatide) insurance requirements may include the following:

1. No personal or family history of medullary thyroid cancer or Multiple endocrine neoplasia, type II (MEN II).
2. BMI of 30 kg/m2 or above, or a BMI of 27-29.9 kg/m2 with an obesity-related comorbidity.

• ✓ Setup an initial consultation appointment

If your prior authorization for GLP-1 medication is approved by insurance, medication will be sent directly to your local pharmacy. Each month you will be billed monthly for refill review by our medical provider who will send prescription refills to your local pharmacy. Once you are ready for a refill, use this Refill Request Form.
